Enrollment Forms

ATTENTION TRIBAL MEMBERS:  A deadline to change or update your addresses has been implemented by the Finance Department for the upcoming per capita payment distribution. The last day to change your address is on or before June 14, 2013.  This action is necessary to ensure that your per capita payment is mailed correctly. The change of address forms are available below. Additionally, if you have recently had a name change due to marriage, divorce, etc., the Enrollment Office must have your marriage license or divorce decree or current drivers license or state issued identification card or court order and your new social security card and a completed Form W-9 in order to change your name in your enrollment record.
