Preventive services requested consist of blood glucose monitors with testing supplies, walking or diabetic shoes, and prescription eyewear assistance, medication assistance not on IHS or Tribal Clinics Drug Formulary and denture assistance. Clients who do not have access to alternate resources will not be denied services pending eligibility and funding availability. The Otoe-Missouria Diabetes Program will not be responsible for payments of unauthorized visits, services or products. Pre-approval of services and products are required if funding from the Otoe-Missouria Diabetes Program is sought.
